You will work closely with a multidisciplinary team to assess plan implement and evaluate patient care plans. Your primary goal will be to promote the health and well-being of our patients and support them in achieving their optimal level of independence.Key Responsibilities:- Conduct initial and ongoing assessments of patients physical emotional and psychosocial needs- Develop and implement individualized care plans based on patients needs and goals- Administer medications and treatments as prescribed by the physician- Monitor patients progress and make necessary adjustments to care plans- Educate patients and their families on disease management medication management and self-care techniques- Collaborate with other healthcare professionals to ensure coordinated and comprehensive care- Provide emotional support and counseling to patients and their families- Maintain accurate and up-to-date patient records- Adhere to all professional ethical and legal standards of nursing practice- Participate in ongoing training and development opportunities to enhance skills and knowledgeQualifications:- Bachelors degree in Nursing from an accredited institution- Valid nursing license in the United Arab Emirates- Minimum of 2 years of experience in home health or community nursing- Excellent communication and interpersonal skills- Strong critical thinking and problem-solving abilities- Ability to work independently and as part of a team- Compassionate and empathetic nature- Proficient in using electronic medical records systems- Fluency in English both written and verbal- Knowledge of Arabic is an assetWorking Conditions:This is a full-time permanent position based in Dubai United Arab Emirates.
